摘要:
目的 比较圣乔治呼吸问卷(SGRQ)和慢性阻塞性肺疾病评估测试(CAT)量表评价慢性阻塞性肺疾病(COPD)稳定期患者健康相关生活质量的差别。方法 收集COPD稳定期患者248例,根据病情分为4组患者(Ⅰ~Ⅳ级),所有患者在入组时、第3个月末、第6个月末分别行SGRQ评分、CAT评分和肺功能检查。结果 入组时4组患者间SGRQ评分、CAT评分均存在显著差异(P<0.05); SGRQ总评分Cronbach’s á系数为0.86,CAT总评分Cronbach’s á系数为0.80;CAT总评分与第一秒用力呼气容积占预计值百分比(FEV1%)之间的相关性(r=-0.78)优于SGRQ总评分与FEV1%之间的相关性(r=-0.57)(P<0.05);第3个月末、第6个月末的统计数据也得到相似的结果。以SGRQ评分有意义最小临床变化值(±4)为标准,CAT总评分变化绝对值≥4时评估患者发生临床变化的敏感性和特异性最高(Youden指数为0.65)。结论 SGRQ和CAT对于COPD稳定期患者健康相关生活质量评估无显著差别,但是与SGRQ相比,CAT内容少、易于完成,更加适合COPD患者使用。
Objective The aim of this study was to evaluate the health-related-quality of life of stable Chronic Obstructive Pulmonary Disease (COPD) patients and compared the self-administered version of the SGRQ (St. George’s Respiratory Questionnaire)and the CAT(Chronic Obstructive Pulmonary Disease Assessment Test) with respect to reliability, validity, and sensitivity to changes. Methods 248 patients with stable stage of COPD were enrolled into this trial, and divided into 4 groups(Ⅰ~Ⅳ) according to severity of COPD. All patients were asked to finish the SGRQ, CAT and lung function test at baseline, 3 months and at 6 months. Results At baseline, the SGRQ and CAT scores showed significant differences between 4 groups patients(P<0.05). The Cronbach’s á coeffcients was 0.86 for the SGRQ and was 0.80 for the CAT at baseline. The Pearson correlation coeffcients between CAT and forced expiratory volume in 1 second of predicted (FEV1%) was -0.78 and was -0.57 between SGRQ and FEV1% at baseline(P<0.05). The data at 3 months and 6 months was similar with that at baseline. Taking the value of minimum clinical changes(±4) of SGRQ score as the standard, the sensitivity and specificity is the highest when absolute value of CAT total score change is ≥4 for evaluating the clinical changes in stable COPD patients (the Youden index is 0.65).Conclusion For evaluating the health-related-quality of life of stable COPD patients, the different between the SGRQ and the CAT showed no statistical significance. However, compared with SGRQ, CAT is less content and finished easily, so it is more suitable for stable COPD patients.
