外周血炎症因子变化与术后认知功能障碍关系的研究

摘要:
目的 研究术后认知功能障碍（POCD）与外周血炎症因子之间的关系。方法 选择全麻手术患者，在术前1 d和术后7d采用成套神经心理测验（Battery of Cognitive Assessment Instruments，BCAI）评价患者认知功能。根据是否发生认知功能障碍分为认知障碍组（n=17例）和非认知障碍组（n=23例）。在认知功能测试的相应时间点分别采血，检测IL-1β、IL-1ra、IL-6、IL-8、TNF-α、TNF RⅡ。结果 POCD 组术后IL-6、IL-8、IL-1ra、TNF RⅡ与术前相比有显著差异（P<0.05）。IL-1β、TNF-α表达不明显。结论 POCD的发生与血浆中促炎因子IL-6、IL-8表达增强和抑炎因子IL-1ra、TNF RⅡ表达减少有关。
Objective　To investigate the relationship between the expression of inflammatory cytokines in plasma and POCD in middle and old aged patients undergoing major surgery. Methods　40 patients undergoing major surgery under general anesthesia were checked cognitive score with BCAI at 1d preoperation and 7d postoperation. Depending on whether the occurrence of cognitive dysfunction, the patients are divided into cognitive dysfunction group (n=17) and non-cognitive dysfunction group(n=23) . The levels of IL-1β、IL-1ra、IL-6、IL-8、TNF-α、TNF RⅡwere detected according to the time set in advance. Results　Compared with control group, levels of IL-6 and IL-8 postoperation were significantly higher in POCD group ( P < 0.05) , therefore levels of IL-1ra and TNF RⅡ were significantly lower( P < 0.05) . Conclusion old aged patients with POCD are associated with high expression of IL-6, IL-8 and low expression of IL-1ra , TNF RⅡ.
